State Innovation Model (SIM)

The Centers for Medicare and Medicaid Services awarded the State of Michigan with funds to design a State Innovation Model (SIM) for health system transformation that improves health outcomes and system performance and reduces costs. Michigan’s SIM initiative is working toward realizing Governor Snyder’s vision for a healthier, more prosperous state. The foundation of this transformative effort is a primary care-based system that leverages the functions of a patient-centered medical home, where care is coordinated, patients receive preventive services, electronic health records are utilized, and health information is shared in a secure and efficient manner to maximize quality and reduce costs. MPHI is providing project management support and content development services.

To support this transformation effort, a 29-member Advisory Committee comprised of representatives from payers, health systems, universities, providers, consumers, businesses, and community organizations was convened to advise MDCH’s SIM Management Team on its transformation efforts.  The final deliverable will be a State Health Care Innovation Plan that will provide a roadmap for transformation, encompassing the Advisory Committee’s six goals:

  1. Strengthen the primary care infrastructure to expand access for Michigan residents
  2. Provide coordinated care to promote positive health and health care outcomes for individuals requiring intensive support services
  3. Build capacity within communities to improve population health
  4. Improve systems of care to ensure delivery of the right care, by the right provider, at the right time, and in the right place
  5. Design system improvements to reduce administrative complexity
  6. Design system improvements that keep insurance premiums affordable for individuals and employers
