Partners in Care Concierge
MPHI received a grant from the Michigan Health Endowment Fund for the purposes of expanding, enriching, and evaluationg Jewish Family Services of Washtenaw County’s (JFS) Partners in Care Consierge (PICC) program. PiCC, originally developed in 2008 by JFS, is an innovative “door-through-door” patient appointment accompaniment program to help older adults age in place with health and dignity. As part of the program, JFS provides medical transportation and pairs clients with trained volunteers from the community who support patients during provider visits. As part of this grant, MPHI is conducting a rigorous evaluation including a randomized control study to assess PiCC’s ability to impact service utilization, cost, and clinical outcomes.
